How To Schedule Your COVID Vaccine Appointment

1. Press the button below and select a location from the options to view any available appointments. Appointments will only become visible 7 days prior to the vaccine clinic day and we will use our social media page to to publicize any clinics that are planned for the week so that you know when to look for appointments.

2. Once you have booked your appointment, you will receive an email confirmation for your scheduled appointment shortly after booking.

3. Reminder Emails and Text Messages: You will receive a few notifications reminding you about your appointment time leading up to your appointment. You will receive a TEXT and EMAIL reminder about your scheduled appointment time 2 hours before your appointment as well as an EMAIL reminder 24 hours before your scheduled appointment time.

4. Once your appointment has been booked, please print and complete the intake form that is found in the confirmation email and arrive to your appointment with a photocopy of your photo ID and insurance card. If you are unable to arrive with a photocopy, most of our clinics are equipped to make a photocopy for you.

Second Dose Appointment: At your first dose appointment you will receive a vaccine reminder card which will have a date on the back for your second dose. Please keep this card and return on that date at the same time and place as your first appointment.

How to Cancel or Reschedule Your Appointment

If you need to cancel or reschedule your appointment, PLEASE do so by going to the confirmation email and selecting the red button that says "Change/Cancel Appointment"

You will be brought to your appointment confirmation page, where you will be given options to cancel or reschedule your appointment.
